To qualify: Must be 18 years of age or older. Must be considered in medically acceptable condition.
Weight loss results must be turned in by Thursday, March 31, 2016 by 6pm. Weigh-ins can only be done in Inshapemds clinic. All program payments must be completed prior to March 31, 2016. Participants must partake in InShapeMD weight loss program. Programs vary depending on amount of needed weight loss. All programs will include but are not limited to: Initial consult, comprehensive blood work, physician appointment, and orientation to program, weekly weigh-ins, a food journal, and full program manual.
Ways to Win:
Register with InShapeMD by purchasing a qualifying weight loss program. Your name will be entered to win by registering. For every 10 pounds lost, your name will be entered again.
Lose all the weight you can by March 31, 2016 to have your name entered to win $1,000.
